How the LTC@Home Consulting Program Works
LTC@Home is an innovative program that brings long-term care services directly to patients in the comfort of their own homes. Designed to support aging in place, LTC@Home provides personalized care coordination, medication management, and remote monitoring to help individuals maintain independence and avoid unnecessary facility placement. It’s a proactive solution for patients, families, and caregivers seeking quality care without leaving home.

- Complete 8-week coaching program: Consists of 1-hour weekly sessions designed to walk you through each part of a LTC@Home Program
- Access to Member only Portal: You will have access to template forms, template policies and procedures, template marketing documents and reference documents and citations
- Access to member only application and emerging topic webinars
- Access to member only platform for peer-to-peer learning
How the Remote Therapeutic Monitoring (RTM) Program Works
A Remote Therapeutic Monitoring (RTM) program helps patients manage chronic conditions more effectively by remotely tracking their response to therapy between office visits. Using digital tools such as apps, wearable devices, or smart pill bottles, RTM programs monitor things like medication adherence, respiratory function, musculoskeletal activity, and treatment response. This real-time data enables healthcare providers to make timely adjustments to care plans—ultimately improving outcomes, reducing hospitalizations, and enhancing the patient experience.
Pharmacy Involvement and Compensation
Independent pharmacies play a key role in facilitating RTM programs by:
- Enrolling eligible patients and educating them on how to use the monitoring tools
- Collecting and transmitting therapy-related data to the appropriate care team
- Supporting medication adherence and responding to therapy issues early
For their role in supporting and coordinating the RTM process, pharmacies may receive an administrative fee. This fee compensates the pharmacy for their time and effort in onboarding patients, monitoring engagement, and facilitating data exchange—all of which are critical to the program’s success and compliance with billing requirements.
By participating in RTM programs, pharmacies not only improve patient care but also generate a new, recurring revenue stream that reinforces their position as a trusted healthcare partner.
